    First picture, just looks like the pump has broken.

    second pic, a standard informal settlement connection,

    third pic, a standard temp. connection on a building site,

    fourth pic, you might just have a little heat build up on the metal between the phases.

    fifth pic, a rush job in a sub station,they just need to tape the exposed connections.

    and the last pic, typical low cost house installation, just the plug lable missing on the DB, the wires below the DB are not part of the elctrical installtions so you dont need to worry about it and ther eis an earth leakage so it should be OK.
